The 2020-2021 recruitment and match season was exceptional. Like most processes affected by Covid-19, this recruitment season is intended to be completely virtual. While some industries have simply moved online, the once ubiquitous in-person medical conversations have always been considered essential and sacred. While the virtual interview has its advantages, the inexperience of visiting the institution in person ends up in a recruitment process with a matching algorithm, which in turn indicates where candidates will spend the next 3-7 years of their lives. It's hard to understand.

In addition to stress for both applicants and programs, the medical residency admissions process is competitive, expensive, and time-consuming. Students say they have taken out up to $25,000 in additional loans to pay for application fees as well as airfare and other travel expenses. Then they chat and try to find a match. While the match rate for national MD graduates is often in the mid-90% range, securing residency is a bit more difficult for DO graduates. For students graduating from medical school outside of the United States, known as International Medical Graduates (IMGs), this is more difficult.

For residency programs, as many as 1,000 applicants are sometimes interviewed to fill ~20 residency positions. Costs increase by approximately $250,000 per program per year during faculty and administration tenure.
Now add the global pandemic and the first season of virtual recruiting in the history of graduate medical (GME). Everyone involved in this year's recruiting season was in unique, uncharted territory. How does this imaginary process affect stress and anxiety?
Early in the outbreak, this was due to the high volume of virtual interviews, leading to "clustering" of interviews between the "best" applicants and a significant overlap of interviews between programs, resulting in an unusual number of applicants and free programs. Notices from governing bodies and governing bodies, including the American College of Surgeons website about the "match crisis" and a letter from the Association of American Medical Colleges (AAMC), were issued during the trial by March 9.

What this means: The match was historic in terms of the number of candidates who entered the match and submitted rankings, the number of programs and positions available, the total number of related US MDs, US DOs, US Citizen IMGs and Non-US Citizen IMG Groups ranked together. The overall proportion of synchronous applicants (including within each subgroup) declined slightly, with the largest decrease among non-US IMGs (down 6.5 percent). The amount increased more than the number of vacancies. In the NRMP press release: "The lack of physician licensing studies in the early stages of the pandemic, coupled with ongoing changes to the assessment and management of these studies in late 2020, has created major challenges for IMG this year and likely contributed to the decline." In addition, changes in clinical rotations may affect the range of correlations.
Our analysis included the top 9 housing professionals as outlined below. In terms of NRMP data for these majors, there were no significant changes in vacant programs and/or positions. In some cases, many specialties are slightly better.
After interpreting the results, we decided to analyze the recruitment interview data to better understand what happened.

In our December 2020 blog post, Thalamus reported that based on all available data, the virtual interview period showed no significant difference in residency program interviews in 9 major specialties (Internal Medicine, Family Medicine, Anesthesiology, Psychiatry, obstetrics and gynecology, pediatrics). surgery, orthopedics and emergency medicine). If an individual chooses any two residency programs in a specialty, the average number of applicants who interview in both programs will be between 5 and 15 percent (although rates vary by specialty). Since our sample covered 20-50 percent of each of the programs we studied and more than 34,000 applicants, we performed several checks to ensure that the data was generalizable (this applies to the entire population of programs) and reliable.
Why This Year's Match® Will Be Strikingly Different
Our first analysis concerned invitations to residency programs and invitations to interviews received by candidates during the recruitment period. At the time, we felt it was too early in the calendar to reveal exactly what was going to happen in the match, given the cancellation of the interview and the fact that we had not yet calculated the speed target. So it can be seen that this year, although interviews were actually conducted, the distribution of invitations to interviews was almost the same as last year. Yes, there have been some applicants who have received over 50 invitations for interviews, but that was only a fraction of the number of foreign applicants in previous years. Average score averaged in single digits, again in line with previous recruiting seasons.
Just like our system, the last interview is underway and the season ends with the submission of the leaderboards on March 3.
In 2021, we began to analyze overall interview completion and cancellation rates across programs, and our findings were as follows.

The slight decrease in the match rate reflects the disproportionately high number of candidates applying for a match compared to the rate of increase in residences, with more candidates being found than ever before.
And so, overall, the pace of the match remains as it has been in recent years, led by the use of a stable Nobel Prize-winning matching algorithm.
Of course, there are many factors here, including where candidates and programs decide to connect and with whom accordingly. Similarly, IMGs have been particularly vulnerable to a visa ban this year, leading to a significant drop in production. Challenges remain, including increasing the number of apprenticeships available to meet the continued acceleration and growth in applicant numbers, and the data will help expand this work.

Overall, virtual interviews didn't have much of an impact on the prevailing interview challenges at GME, and the much-publicized "match crisis" seemed to have been eliminated. However, the process has shed considerable light on an ongoing systemic problem: due to a lack of transparency, applicants continue to submit too many applications and residency programs continue to interview too many, creating a costly and stressful process for everyone. Data and technology could change that in the future as Covid has left a significant mark on the recruitment and outreach process of medical residents.
