Realistically, it is impossible to gather all the knowledge of ophthalmology in the next 1-3 months to do well in the exam. Hopefully this time will be spent reviewing and consolidating previously learned concepts rather than learning concepts for the first time!

Among the many crazy ideas and projects running through my head, I wanted to create a daily study calendar.
I have received a lot of feedback over the years that one of the main challenges in learning ophthalmology is the lack of complete volume and overall structure for comprehensive study of the material. Bhullar and Venkateswaran stated that in a 2022 review article (1). While there are many general reading charts and tips (some of which are free here!), they only refer to textbook pages, with little guidance on how or what to study.

Although this is a large undertaking, I started thinking about how to create a step-by-step training program for pre-ophthalmology (PGY-1) or first-year ophthalmology (PGY-2) residents. My goals were:
The result is a 35-week study calendar with most ophthalmology subjects covered 2-3 times. This allows residents planning to take their exams in mid-March to begin their studies in early July and review the academic calendar within 2-3 weeks.
Because learning is an iterative process, the first four weeks are "Introduction to Ophthalmology" and cover potential visual pathways, eye movements, ocular anatomy and function, and ocular anatomy and function. These grounds will be reviewed in the coming weeks. Dive into the most common diseases in ophthalmology (arranged chronologically), then dive deeper into each subspecialty's top-performing topics.

Of course, the challenge with this approach is that textbook connections can be diffuse and over-abundant. After all, some parts can cover almost a hundred pages of a textbook in a single day, which is definitely not a feasible possibility.
So to make each day's reading more accessible (and as detailed as possible), I've created a sample chapter to see if it might be helpful.

Binocular (fusion) eye movements are synchronized eye movements that help maintain a clear and coherent image despite having two eyes, 12 extraocular motor units, and six cranial nerves all controlling it.
My residents often find binocular eye movement problems and the systems that regulate fusion to be one of the more challenging problems in ophthalmology and neuro-ophthalmology. Symptoms are often vague and difficult to describe. Furthermore, assessment and description of abnormal binocular eye movements are often subtle or difficult.
Although canalization (eye rotation) and alignment are mostly well understood, binocular eye movement abnormalities can affect vision, such as strabismus or gaze palsy.

While basic and clinical science courses explain these systems in detail and show the pathways of each system (important for localizing lesions), I typically teach residents to account for these movements based on what the eyes and head are doing. , what the object of interest is doing and how fast the movements are. All of these movements are supranuclear (ie, the signals controlling these movements begin before the cranial nerve nuclei are activated), so diseases that cause abnormalities in these eye movements affect cortical or brainstem structures, not peripheral nerves or muscles.

We had a great live session where we learned concepts from the BCSC Basics textbook and answered some great questions – prompting this month's live session where we discussed how to learn from our clinical encounters.

So this got me thinking, can we create a community of trainees that can all learn ophthalmology together and provide useful learning tools and advice that people have been asking for?
After hours of planning and work, I can't wait to share this new course/community with everyone! The course is called "How to Learn Ophthalmology". I integrate various learning strategies and proven methods to understand the learning process in ophthalmology. Along the way, we'll discuss different learning tools, consider different programs, and create a personalized curriculum and schedule. I wrote a transcript for each lesson (24 separate lessons). I have a few videos to make and edit, but I want to let everyone know about the course!

But the course is just the beginning! In addition to having access to the curriculum (which I designed for medical students or early residents), I also offer monthly one-on-one sessions where I discuss teaching methods, review specific topics, or learn about my own methods. Learning with practice questions (or anything else that might be useful). Live sessions are also archived for later viewing!
You can also connect with other ophthalmology interns and share tips and resources. If I can attract at least 100 people to the community, I should be able to give up my clinical duties and devote more time to developing useful resources. We already have a great group in the community, I look forward to seeing you there!
I managed to grab a copy of the latest BCSC edition (2022-2023)! Thank you to the AAO and for the privilege of serving on the Practicing Ophthalmology Education Advisory Committee! I will review each book and provide some comments.

There are many different ways to study ophthalmology these days. There are more textbooks and reference books now than ever before; Podcasts, YouTube channels and many other forms of media also provide new streams of innovative content.
Over the past 6-7 years since starting Ophthalmology Review, I've enjoyed experimenting with a variety of ways to create useful content, from directly translating residency notes to creating tables and vignettes, writing review articles, and more. Thinking about how to teach the incredible field of ophthalmology.
